Med School NEVER Prepared Me For This

4. Ožu 2020.

Continuing with the medical Reddit thread series this episode is on things that med school didn't prepare us for. In reality medical school can't prepare us for everything but there are some suggestions I have about improving the curriculum. Responding to Reddit threads is a great way to learn and have a good time so smash that like the button to let me know you're on board!
If you have an idea of something you want me to cover in-depth, please let me know because I take your requests seriously. We will be back with more Medical Drama Review/Responding to comments Series in a couple of weeks, so please submit more names of shows/questions you'd like for me to watch/answer. Love you all!

  • My great nan wroked in a hospital, in the morgue part and she was just working and she was wheeling someone down the corridor to the fridges and the body sat up (because of all the gas still inside the body) she screamed and started running down the corridor, she knew why it happened, but it was still terrifying for her.

  • When my daughter was in 1st grade she had a rare form of cancer. Of course she was so terrified of everything going on, we had been living at the hospital going through tests for days before the actual surgery was done so she was tired of being pricked and prodded, that she fought getting the shot before the surgery. The nurse and I held her down while the intern was to give her the shot. She hesitated and got yelled at by the nurse in front of me. She then did the shot. Later while my daughter was still in surgery the intern came to apologize to me. She said she wanted to become a nurse to help children. She had never considered that the children didn't want that help. She said she was reconsidering her choice. At least my daughter taught her right away what the truth is.

  • When you mentioned doctors needing more education in disabilities... yes. It took until I was nearly 20 to be diagnosed with ASD. Apparently this is the case for many people, as doctors are still learning about the disorder and every year they debunk more misinformation from inaccurate studies. It is so hard to get the correct diagnosis for ASD, as even if you fit every part of the criteria for diagnosis, a lot of doctors are not up to date on recent discoveries- still believing it only appears in one way, boys only, etc... And if diagnosed late, people may have already aged out of most of the resources.

  • Retired OB nurse here. Many years ago we had a patient with an underlying immune disorder who had gotten pregnant despite it. (Not necessarily a planned or deliberate pregnancy, but a very welcome one to her and her husband.) She had come in to the Labor and Delivery suite with shortness of breath, was stabilized, and sent to the ante/post partum floor. She ended up arresting while there. My charge nurse sent me and another L&D nurse up to assist as much as possible with the code, to free up the regular nurses on that floor. The patient, a lovely woman, ended up not surviving. All the various and sundry people on the code team left after her death was called. and my fellow nurse and I were left to prepare her body for transfer to the morgue. It was not something I'd done before, but absolutely part of my job, and one I was willing to do, particularly as the patient was also an employee at the hospital, whom I knew. Giving her the most respectful and reverent "send off" was the least I could do. BUT, the part that upset me horribly was how the code team handled the immediate aftermath. In the process of the code they'd basically completely uncovered her to have access to veins for IV lines, her chest for compressions, etc. It was appropriate that during the code she was essentially naked on the bed, no problem there. BUT, when they "called" the code, they all just left, leaving her lying, completely naked, on the bed. Really? They couldn't take one second to pull a sheet up over her? That was 35 years ago, and it haunts me still.

  • A lot of the nurses I know are overweight and stressed out. There is a small clinic in the store I work at and these nurses subsist on sugar. Lots of it. I never comment...my mind is on other things, like processing their transaction when they come through my line...but they are always saying, "Yeah, sorry, not healthy, but I need the energy!" I don't give my real thoughts on the matter because, well, they are _nurses._ They know already very well what the deal is with a poor diet, chronic stress, and bad sleeping habits.

  • When I went to the gynecologist a couple of weeks ago my doctor came in and was joking around with me and my dad, she cussed a few times, she was laughing but was also giving me the time to explain what I had wanted out of the appointment as well. She was extremely calming to be around because you expect doctors to be very serious and very uppity but she reminded me a lot of some of my friends. She wasn’t talking down to me when I didn’t know exactly what certain birth controls were and when I told her I wanted to get the implant and not the pills which is what is normally what you would do first. 10/10 best doctor I’ve ever had. Only medical personnel that I’ve had time with that have acted on the same level is a nurse that I had last December. She told the nurse she was transferring me to that I was an easy patient since I never asked for anything but ice water and we laughed for a bit about it.

Looked at the nurse. The nurse goes out into the hall to tell her daughter her mother is alive. They go back into the room. Tell mom. She screams. Dies. Did the news kill her? What happened?

  • For ALL Healthcare workers from Doctors to janitors, NEED to be trained on Patient Modesty. It is NOT only women who are modest. And when a male patient says NO female healthcare workers THAT SHOULD be treated the SAME way a female patient saying NO male healthcare workers... . STOP immediately verify with the patient what are their wishes then DO THEM, and if you cannot explain the situation, for example . Mrs. Smith, I understand you want all female healthcare workers, we will do everything we can to accommodate your wishes, however we do not have a female doctor available at the moment, she will be here for morning rounds, would you like to wait or proceed with a male doctor, with female healthcare workers present? If she wants to wait, tell her you understand and will do so, but also explain the possible (if any) outcomes are possible with waiting for the female doctor.... . And . An abused patient, or respect for their wife/husband who are the only person of that gender to see them completely nude. It is not misandry from female patients, or misogyny from male patients... . • From a patient perspective it is kind of scary when doctors don't know what is going on and tells you "If this doesn't work you will be admitted, as we don't know what is wrong". I had chest pains, fever, couldn't breathe properly and couhing so they tought I had pnemonia but they couldn't confirm it due to all tests coming back negative and by this point I had been in and out of hospital for a week for different kinds of tests. So they gave me antibiotics and just hoped it would work. It turned out that I had pnemonia and have had it 3 years in a row always in the summer and it never shows on any tests and doctors don't believe me when I tell them what it probably is.

In final year, there are days that you have to do ward work, and at night stay on call, and in the morning you go to do ward work again without proper resting. • The one about not always having the answer reminded me of my experience as a child being diagnosed with an Arteriovenous Malformation. My GP said it was just growing pains first, then an overgrown muscle from the leg compensating for an issue with the other leg, to a tumor, to finally the malformation. We went through 3 different doctors before one thought he knew what it was. He referred us to a 4th who finally diagnosed it. I guess this particular type of malformation is usually on the brain/head and doesn't present until later in life. Mine was in my left thight, woven in among the muscles there. The increase in hormones triggered by puberty at 10 caused it to start growing. It's unusual placement and presentation made it difficult to identify and it took us almost 2 years to get an answer.

Eg. patient multiple medical issues such as metastatic cancer, AKI and/or chronic kidney disease, previous MI’s with AF or other cardiac issues all wrapped into one very sick package. And they are usually old which decreases the odds even further that any resuscitation attempts will be anything but brutal, unsuccessful and traumatic for everyone involved.
